In 1916, a Russian immigrant named Herschel Danielovitch celebrated the birth of his son, Issur. Being new to America, the family adopted the last name, Demsky, and Issur started going by "Izzy".
Izzy Demsky ended up becoming an actor, and changed his name again; this time to "Kirk Douglas." It was a change that he later admitted that he wished he could take back. But, he became successful, and later had a child, Michael, who would go on to his own successful Hollywood career as Michael Douglas.
A short time later, in Pennsylvania, another guy set his sights on being an actor. His name was also Michael Douglas. But the Screen Actors Guild doesn't allow duplicates, so he had to change his last name for his acting roles. He picked Keaton.
So the Michael Douglas, whose father wishes he would have been Michael Danielovitch, or maybe Michael Demsky, acts under the name Michael Douglas, and the Michael Douglas from Pennsylvania, whose father presumably is comfortable with him being Michael Douglas, acts under the name Michael Keaton, but conducts all non-acting business as Michael Douglas.
