Grave of the Fireflies animated film from the Miyazaki studio is probably the saddest I can remember. Two young kids try to live among the radiation immediately after the nuclear bombs drop. It's such a downer I doubt if it could have even existed if it was real people and not cartoons.

Mine would be a couple struggling for years to have a child and finally when they can the mother dies in the delivery room.

Cancer babies and their mothers on a bus trip to a new clinic with a groundbreaking cure but enroute the bus swerves to miss a sack of kittens some bozo threw on the road and ends up crashing into the very same clinic where the cure is destroyed forever. And THEN the sack of kittens gets hit by a semi truck.
